How to Contact

Since 1984, our Sister Hyacintha Program has been bringing our high quality, compassionate medical care to the homebound. We continue the work begun by Sister Mary Hyacintha of the Sisters of St. Francis of Philadelphia over a century ago. We realize that often the one factor that will enable someone to remain in their home is having access to medical care. This unique program enables participants to remain independent because it provides that access.

The Sister Hyacintha Program offers you:

· Excellent medical care provided in the comfort of your home
· Routine medical check-ups and sick call visits
· A doctor who specializes in home care
· Nursing case management to oversee all your health needs
· Pastoral Support
· Laboratory Services
· Prescription Renewal

 How to Contact

Patients who contact our Sister Hyacintha office will be offered an appointment for one of our specially trained nurses to come out to the home to determine specific needs and explain the program in detail.  How Are The Services Paid For?

No payment is made at the time of service.  St. Francis Medical Center will mail you a bill for all fees not covered by Medicare, Medicaid or billable secondary insurance.

  • Medicare Part B pays for 80% of the doctor home visits and other covered services.
  • Medicaid insurance pays 100% of the doctor home visits and other covered services
  • Commercial Insurers may provide partial or complete reimbursement
    Self Pay patients are also accepted
