DAISY Award

The DAISY (Diseases Attacking the Immune SYstem) Award is an international recognition program that honors and celebrates the skillful, compassionate care nurses provide every day. The DAISY Foundation was established by the family of J. Patrick Barnes after he died from complications of the auto-immune disease ITP in 1999. During his hospitalization, they deeply appreciated the care and compassion shown to Patrick and his entire family. When he died, they felt compelled to say “thank you” to nurses in a very public way. Please say thank you by sharing your story of how a nurse made a difference you will never forget!

Congratulations BPCI Team!

The Trinity Health Leadership Team recently awarded its "Excellence In People-Centered Care Redesign" to the SFMC Bundled Payment for Care Improvement Team under the leadership of C. James Romano, MD and Debra Birkenstamm. This award recognizes the Teams contribution to significantly impacting the care of the SFMC patients enrolled in this value-based program.

"Our innovative initiatives are the direct result of working in a multidisciplinary framework of care design with our SFMC teams in order to improve the cost of care and outcomes for the population we serve in our community", stated Debra Birkenstamm, Corporate Director Population Health.

The BPCI program will be advancing to a new model of care from CMS in the fall of 2018. This will result in the expansion of our current diagnoses and allow us to foster an affordable, accessible patient-first healthcare system for an even larger number of our patients.

Congratulations to BPCI, Case Management, Physical Therapy, Cardiovascular Service and our CDI teams for their awesome work!
