Welcome to the St. Francis Diabetic Self-Management Program. This free program is designed to provide you with the tools and skills needed to take control and manage your diabetes. This program is designed to create better care for individuals including access to care, quality of care, health outcomes, and improved health.

Group visits; providing the tools

  • You can enroll on your own or be referred by your healthcare provider—there is no cost for this service.
