What is "DSRIP"?

The Delivery System Reform Incentive Payment Program (DSRIP) is a demonstration program designed to create better care for individuals including access to care, quality of care, health outcomes, and improved health for the population, and lower costs by transitioning hospital funding to a model where payment is contingent on achieving health improvement goals. The DSRIP program supports the Healthy New Jersey 2020 vision: "For New Jersey to be a state in which all people live long, healthy lives." The federal government has approved the NJ DSRIP program.

Why is it important to the Trenton community?

St. Francis Medical Center and our Trenton Health Team partners, Capital Health, Henry J. Austin and the City of Trenton, conducted a Community Health Needs Assessment in 2012. The care and treatment of chronic diseases were identified by our community as a high priority need. Diabetes has been specifically identified as one of the chronic diseases requiring improved management. It is well known that the disease is closely associated with high blood pressure, obesity and heart disease and is one of the leading causes of hospitalization in the U.S. 

How will St. Francis Medical Center participate in the DSRIP program?

St. Francis Medical Center will collaborate with community partners in establishing a new program for the management of diabetes. The project will focus on the identification of individuals who need help managing their diabetes. Services provided will include education on how to best manage this disease, self-care skills, access to providers, and access to other resources to promote the health of the community. The goals of the project are:

  1. Reducing hospitalizations
  2. Reducing emergency episodes
  3. Improving patient ability to manage their diabetes
  4. Improving overall health of the community
